Music therapy

Using rhythm, melody, and song as clinical tools for mood, movement, and communication. An honest look at where the evidence is strong (mood in dementia, gait cueing) and where it is still emerging.
Delivered by trained therapists, music-based approaches probably improve mood and behaviour in dementia and can pace movement (gait) in Parkinson's. Effects on memory and long-term change are limited - it works best as a meaningful add-on.
Encouraging results for some goals, but the evidence is still limited or lower quality.
Music reaches the brain through many networks at once - which is why rhythm and song can help movement, mood, and speech even when other routes are blocked.
What it is
Music therapy is the clinical use of music - listening, singing, moving to a beat, or playing - by a trained therapist working toward specific goals. In neuro-rehab it usually takes one of a few evidence-based forms: rhythmic auditory stimulation (a steady beat to pace walking), melodic intonation therapy (singing to support speech in some non-fluent aphasia), and music-based activities for mood and connection in dementia.
What the evidence says
The strength of evidence depends on the goal:

- A Cochrane review found at least five sessions of a music-based intervention probably improves depressive symptoms and may improve behaviour in dementia, with little effect on cognition [van der Steen JT 2018] .
- A meta-analysis showed rhythmic auditory cueing improves gait (speed, stride length, cadence) in Parkinson’s [Ghai S 2018] .
- WHO guidance supports offering meaningful, person-centred activities - including music - as part of dementia care [World Health Organization 2023] .
Honest limits
- It is not a replacement for physiotherapy, speech therapy, or medical care.
- Benefits for mood often fade without regular sessions.
- The most reliable ingredient is personally meaningful music, actively used.
