Energy conservation and pacing
Practical techniques for managing fatigue after neurological illness - pacing your day so you can do more of what matters without paying for it later.
Fatigue after neurological illness is different from ordinary tiredness. It doesn’t fully resolve with rest, can be triggered by very little activity, and can wipe out the rest of the day. Learning to work with your energy, rather than fighting it, is the foundation of energy conservation.
The energy envelope
Think of your daily energy as a fixed budget. Neurological fatigue typically means the budget is smaller than before - and overspending it (called a boom-bust cycle) leads to a crash that can last hours or days.
The goal is to stay within your envelope, distribute activity more evenly across the day, and gradually expand the envelope through steady, sustainable practice.
The 4 Ps of energy conservation

| Planning | Think ahead. Schedule demanding tasks for your best time of day. Build in rest before and after. |
| Prioritising | Identify which activities matter most. Let go of, delegate, or simplify lower-priority ones. |
| Pacing | Break tasks into smaller chunks with rest between. Don’t push through fatigue - stop before you crash. |
| Positioning | Sit rather than stand when possible. Use good posture and ergonomic set-up to reduce effort. |
Practical pacing techniques
Activity monitoring Keep a diary for a week noting what you did, how much energy it used (out of 10), and how you felt afterwards. Patterns often emerge: particular times of day, tasks, or environments that cost far more than expected.
Planned rest Rest proactively - before fatigue hits, not after. A 10-15 minute rest between activities can prevent a full crash. Rest means lying or sitting quietly, not scrolling a phone (screens are cognitively effortful).
Task modification
- Shower seated rather than standing.
- Prepare meals sitting at the kitchen table.
- Use a trolley to move things around the house rather than carrying.
- Use voice control on your phone for tasks that need fine motor effort.
The two-minute rule If something takes less than two minutes (and you have the energy), do it now. Deferring small tasks creates a backlog that costs more later.
When energy management is different from giving up
Energy conservation is not a permanent ceiling. The goal is to:
- Participate in rehabilitation and daily life consistently, because consistency drives neuroplasticity.
- Gradually increase activity tolerance by never fully depleting the envelope.
- Eventually need less active management as fatigue improves.
Crashing repeatedly, by contrast, reduces participation and delays recovery.
Working with your OT
An occupational therapist can assess your energy use objectively, help you redesign your daily routine, and identify specific modifications for the activities that cost most. They can also help distinguish normal rehabilitation fatigue (which builds tolerance) from fatigue that needs medical investigation.
