What is multiple sclerosis?

A plain-language introduction to multiple sclerosis (MS): what happens in the nervous system, the common symptoms, the main patterns, and how rehab helps day to day.

Multiple sclerosis (MS) is a long-term condition in which the immune system mistakenly attacks the protective coating around nerves (myelin) in the brain and spinal cord. Where the coating is damaged, messages along those nerves slow down or get scrambled - which is why MS can affect so many different functions.

What it can affect

MS is different for everyone, because it depends on which nerves are involved. Common experiences include:

  • Fatigue - an overwhelming tiredness that is out of proportion to activity.
  • Movement and balance - weakness, stiffness (spasticity), or unsteadiness.
  • Sensation - numbness, tingling, or pins and needles.
  • Vision - blurred or double vision, often in one eye.
  • Thinking - slower processing, or trouble with memory and attention.
  • Bladder, bowel, and mood changes.

The main patterns

  • Relapsing-remitting MS - flare-ups (relapses) followed by periods of recovery. This is the most common pattern at diagnosis.
  • Secondary progressive MS - a gradual build-up of disability over time, sometimes after years of relapses.
  • Primary progressive MS - symptoms that slowly progress from the start, without distinct relapses.

How rehabilitation helps

There is no single cure, but a great deal can be done to keep you active and independent. Medication from your neurology team can reduce relapses, while rehab targets everyday function:

  • Physiotherapy for strength, balance, walking, and managing spasticity.
  • Occupational therapy to make daily tasks easier and conserve energy.
  • Exercise - once thought risky, it is now known to help fatigue, mood, and fitness when paced sensibly.
  • Cognitive strategies for memory and attention when thinking feels foggy.
